What is YSR Aarogyasri Health Card Apply Process:

  • Step 1:  Log on to www.ehf.ap.gov.in.
  • Step 2: Click on the Sign-in button for registering.
  • Step 3: Your User ID & Password is available with the STO/ APPO (or) can be obtained from Seva Kendram by dialing ‘104’.
  • Step 4: Getting the User Id and password login into the Official portal.
  • Step 5: After login: change your password
  • Step 6: Select your category from the dropdown list.
  • Step 7: Now click on the registration button.
  • Step 8: If you have Aadhar Card enter the 12 digit number
  • Step 9: Now click on the retrieve details.
  • Step 10: The application form will appear to fill all basic details.
  • Step 11: Then click save.
  • Step 12: Next click on add attachments and upload the required documents.
  • Step 13: Now click on  Add beneficiary to add other members of your family.
  • Step 14: Upload photo & Aadhar Card of family members.
  • Step 15: Verify the correctness of the details.
  • Step 16: ‘Save’ the enrolment form.
  • Step 17: Now ‘submit’ the application by clicking the “Submit application” tab
  • Step 18: After clicking ‘submit application’ option you cannot edit any data
  • Step 19: You will receive the following message on your mobile
  • Step 20: “Application submitted successfully; Your health card enrolment ID:…; Your signed application uploading is pending”
  • Step 21: Now ‘print’ the Application Form
  • Step 22: Sign the printed application and make a scanned copy
  • Step 23: Now you may need to log in again as the session may expire
  • Step 24: Now upload signed application by clicking “add attachment”
  • Step 25: Now ‘submit an application for approval’
  • Step 26: Task will be sent to your STO/ APPO login
  • Step 27: Now you will receive one more message on your mobile, as indicated below:
    • “Application submitted successfully with enrollment ID………… and Pending with STO/APPO Code”
